How do I know if the claim information is accurate?

Each time you receive or are notified that a new Monthly Claims Summary is available, review it thoroughly to verify the key dates and amounts shown are accurate. In particular, compare any amounts you paid out of pocket to a provider to the amounts shown under “Patient Responsibility.”

If you have questions about your claim information or if you think something is missing or incorrect, you can:
