What happens if a claim is pended (not paid) by the LM HealthWorks Plan?

In the event a claim is pended or not paid, you will be mailed an Explanation of Benefits statement stating the reason.

You may also call the LM HealthWorks Plan at 1-877-458-4975. A customer service representative can tell you the status of a claim that you or your provider have filed and/or answer questions about how a claim was processed.
