What if I paid for a covered service that should have been paid by the HealthFund?

Or by the LM HealthWorks Plan?

If it is determined that you paid for a service that should have been paid by the HealthFund or the LM HealthWorks Plan, you will receive a check by mail, reimbursing you for the amount. The check will be accompanied by information identifying the service(s).

  1. Your name and address
  2. Date the check and explanation was printed and mailed
  3. Explanation of service
  4. Amount being reimbursed to the member
  5. Check payable to the member

If you receive a check and have questions, you can:

Call the LM HealthWorks Plan at 1-877-458-4975 and a customer service representative will answer your questions.

Note: New credits added to your HealthFund are not applied retroactively to claims that have already been processed, rather the credit will apply to future claims.
